I am currently working on an application which might be able to help you. And maybe you can help me beta test it... :-)

It is called FlickrFriend. And here is what it does:
1. Pushes all of your iPhoto images to Flickr, including meta data, album structure, tags and faces (so far not the comments, events and geo, but that will come).
2. Uses the capture date to uniquely identify images so you can use the application to sync more than one iPhoto library and still avoid duplicates.

So far downloads from Flickr are not part of the application and nothing is ever deleted on Flickr.
